Any orthodontic appliance, designed to your prescription

Not every appliance fits a named service, and it should not have to. If you can write a prescription for it, it can be designed: a Hawley or wraparound retainer, a transpalatal arch, a lingual arch with a specific feature, a habit appliance, an anterior bite plane, a distalising appliance, a splint with orthodontic components, or an appliance of your own design. The process is the same as for every other service. The appliance is designed from your scans to your prescription, you review it tooth by tooth in the browser and comment on anything you want changed, and the revised version comes back for your approval in 3D. Only then is anything exported, in the format your laboratory prefers, with chairside instructions and material specifications. Where you ask for it, production is overseen so that what is fabricated matches the geometry you approved.

What we need from you

  • Upper and lower intraoral scans as open meshes (STL, PLY or OBJ), with a bite scan or registration
  • Any imaging the design depends on, such as a panoramic radiograph or CBCT
  • Your prescription: the appliance, its purpose, the components you want and where, the materials and gauges you prefer, and anything that must be relieved or avoided
  • A sketch, photograph or reference design where that describes the appliance more clearly than words
  • Your laboratory's fabrication method and file format, and whether you want production oversight
  • A case reference of your own choosing, not a patient name

How this service runs

  1. Prescription review

    Your prescription is read against the scans, and any question about a component, a material or a tooth is raised before design begins.

  2. Design

    The appliance is built from your scans to your prescription, with each component designed against the actual surfaces of the teeth it touches.

  3. Contact and occlusion check

    The design is checked against the retained teeth and the opposing arch, and anything that would interfere is corrected.

  4. Doctor review

    You review the appliance tooth by tooth in the browser and comment where you want something changed.

  5. Revision

    Your comments become a new version. Earlier versions are kept, so you can compare what changed.

  6. Approval

    You approve the version you are satisfied with. Nothing is exported without it.

  7. Export and oversight

    Files go to your laboratory in its format with chairside instructions and material specifications, and, where requested, the fabricated appliance is checked against the approved design.

How the planning is done

What a prescription needs to contain

A good prescription describes the appliance the way you would describe it to a technician you trust: what it is for, which teeth it engages and how, which components it carries and where, what it is made of, and what it must not do. It does not need to be long. A retainer prescription that names the clasp type on each abutment, the labial bow's extent and the acrylic coverage is complete. A habit appliance prescription that describes the crib's position and the teeth it is anchored to is complete.

Where the prescription is silent, the design follows the convention for that appliance type and says so in the notes, so that you can change it before approval rather than discover it at delivery. Where the prescription asks for something the scan does not support, such as a clasp on a tooth with no usable undercut, the design raises it as a question rather than building something that will not work.

A sketch is often the clearest prescription for an appliance of your own design. It is read alongside the scans, and the first version is built as a proposal for you to correct.

Review, revision and approval in 3D

The appliance is reviewed in the browser on the scanned arches, so that you see it on the patient's teeth rather than on a diagram. Comments can be attached to a tooth, so that a note about a clasp is unambiguous, or to the design as a whole. Each round of comments produces a new numbered version, and the earlier versions are kept, so that a change can be compared and, if necessary, reversed.

Approval is explicit. You approve a version, and that version is the one exported. The boundary matters more for a custom appliance than for a standard one, because there is no named service description to fall back on: the approved geometry is the specification.

The chairside instructions and material specifications are written from that approved version. They state the wire gauges, acrylic or resin and metal the design assumes, what to check at delivery, and which elements can be adjusted at the chairside without changing what the appliance does.

Production oversight

A design can be fabricated well or badly, and a laboratory that receives a file it has not seen before may interpret it. Production oversight is the option of having the fabricated appliance checked against the approved geometry before it reaches you: the laboratory's output is compared with the design for the position of components, the fit to the model and the materials used, and any discrepancy is reported before delivery.

Oversight is arranged with the laboratory you nominate and follows its workflow; it does not replace the laboratory's own quality control, and it does not make Alignodontic DPS the manufacturer. It exists so that the appliance in the patient's mouth is the one you approved on the screen.

Prescription

Hawley retainer
A removable retainer with an acrylic baseplate, a labial bow and clasps, used to hold teeth after active treatment.
Transpalatal arch
A wire arch across the palate between the upper molars, used for anchorage, rotation or arch width control.
Habit appliance
An appliance carrying a crib or similar feature to interrupt a habit such as thumb sucking or tongue thrusting.
Bite plane
An acrylic platform, usually behind the upper incisors, that holds the posterior teeth apart to open the bite or free the occlusion.
Version
A numbered state of the design; only an approved version is exported for fabrication.
Production oversight
The optional check of a fabricated appliance against the approved digital design before delivery.

Questions doctors ask

What appliances can be designed this way?
Any appliance you can prescribe: retainers, palatal and lingual arches, habit appliances, bite planes, distalising and other auxiliary appliances, and designs of your own. If a named service exists for the appliance, such as space maintainers, expanders or functional appliances, that service applies; everything else is designed here.
How do I describe an appliance you have not designed before?
Write the prescription as you would for a technician, and add a sketch, a photograph or a reference design where that is clearer. The first version is built as a proposal and you correct it in review.
Do you fabricate the appliance?
No. The deliverable is an approved design exported for your laboratory. Fabrication is your laboratory's work. Production oversight, where you ask for it, checks the fabricated appliance against the approved design; it does not make us the manufacturer.
Which materials do you design for?
The ones your prescription and your laboratory use: stainless steel wire by gauge, acrylic or printed resin, and printed or milled metal frameworks. The materials assumed by the design are stated in the specifications that accompany the files.
Can I revise the design more than once?
Yes. Each round of comments produces a new version and earlier versions are kept. Approval is given to one version, and only that version is exported.
What is in the chairside instructions?
Delivery checks for the appliance, which elements may be adjusted at the chairside and which should not be, and the material and wire specifications the design assumes, so that adjustments and repairs are made with the right components.
